Services:
Cardiff Alcohol and Drug Team is a small specialist Social Work team within Cardiff Council’s Adult Social Services Directorate. The Team’s remit is to undertake Wellbeing Assessments to identify what matters to individuals ,identify their outcomes and what care and support needs they require assistance with under the Social Services and Well-being (Wales) Act 2014 legislation. We work with people whose primary needs are drug or alcohol related.
The Social Workers and Social Work Assistants carry out Wellbeing assessments, which identify the outcome that the individual wishes to achieve using a person-centred approach, which supports identification of that individual’s strength, their support networks, risks and potential barriers to them achieving their outcomes.
The following services are available:
A social worker is also based in the Community Addictions Unit (C.A.U.) led by Cardiff and Vale University Health Board.
The services that may be provided dependent upon the individuals identified outcome include short or longer term professional support – for example in order to access community resources identified to meet the individual’s care and support needs. We can provide domiciliary packages of care to enable people to remain living safely in their own home. We are able to access residential rehabilitation placements for individuals who have engaged in community treatment options whose personal outcomes can only be achieved with the provision of an intensive therapeutic placement.
