Hafod Care is a charitable, "not-for-profit" Housing Association. Since 2002 we have specialised in delivering a wide range of housing, care and support services. We currently support over 1,000 people each year across South and West Wales.
Our services include: •Care Homes for Older People •Tenant Support Services •Home Care •Extra Care Housing •Supported Housing We have over 1,200 dedicated staff and we all share one vision 'Making Lives Better'.
Referrals are made through local Community Mental Health Teams, GPs, family members, probation services, NHS, Local Authorities, self referral, social services, prison or young offenders teams, housing departments, other HAs, internal transfers.
Vale – referrals are via the Supporting People Team.
Young Persons Project Cardiff - 18-25 year olds self referral or via agencies including Social Services and homelessness departments or for young people threatened with homelessness and who require support.
Carmarthen Young Persons static project - referrals via Carmarthen Council's Homelessness department.
Carmarthen floating support project – referrals via Carmarthen Council.
Cardiff 25 project - referrals via Cardiff Council's Tenant Support Team and other agencies including self referrals.
Cardiff Families and Lone Parents project – Referrals via Taff and CCHA.
